As a progressive and vibrant healthcare organization, we are committed to being an innovative center of excellence in community hospital care.Job DescriptionWorking closely with the inpatient and outpatient child and adolescent interdisciplinary teams the Child and Youth Care (CYC) worker will collaborate on treatment plans for patients in the community, conduct home and community visits to address issues such as school compliance, or lack of follow through with office treatment and provide family with support while connecting them to appropriate services.The CYC will work closely with the patient to address any housing issues, financial struggles and ensure that they have adequate skills for daily activities, such as grocery shopping, attending appointments, banking/ budgets, applying for appropriate supports, such as student welfare etc. The worker will accompany patients to school if school compliance is an issue and will work closely with the Mental Health Liaison Nurse/ Social Worker to ensure the patient is receiving appropriate school accommodations.From the inpatient unit, the CYC will assist patients by integrating them back into the school system to ensure a successful transition. For patients involved with our outpatient mental health program who are struggling with socializing with their peers due to their mental illness, the CYC will engage them in social activities such as attending the YMCA.The CYC will work closely with the Child Psychiatrists, Social Workers and Nurses to ensure that progress or delay in treatment is articulated to ensure a seamless transition between the teams. The CYC will also play an important role for patients who attend the Emergency Department in ensuring that the patient and their family are connected with in a timely fashion and supported until engaged to appropriate services.QualificationsHalton Healthcare endeavors to hire individuals who are competent in their chosen profession and who exemplify our mission, vision and values of accountability, respect and compassion which guide our individual behaviours and attitudes.

  • CYW certificate and diploma or degree from a recognized community college or university.
  • 3-5 years of child and adolescent experience preferably in the hospital setting
  • Must have knowledge of children’s psycho-social development
  • Able to work independently and effectively within a team.
  • Must have competency in assessing risk issues and being able to address concerns within the community
  • Experience in working with high conflict families and the skill set in deescalating conflict.
  • Understanding of the school system and support within the Halton community.
  • Must have computer skills (Word Processing)
  • Satisfactory employment and good attendance record
