Job description: JOB SUMMARYThe Clinical Resource Nurse (CRN), as part of the Clinical Management team, and under the direction of their immediate Supervisor, is accountable for the delivery of care and service to clients and staff, while ensuring compliance with internal and external standards of practice and Provincial regulatory requirements. The CRN will be responsible for providing and/or coordinating clinical education, in-services, and supporting branches in delivering high quality client/patient care. As a member of the Bayshore interdisciplinary team the CRN will provide mentoring, education, team building, support, and care expertise to community nurses, unregulated care providers, and other health care professionals and paraprofessionals.DUTIES AND RESPONSIBILITIES· The primary role of the CRN is to facilitate a learning environment where evidence-based knowledge is translated via education to optimize the patient’s outcomes and experiences. This includes developing education plans and activities aimed at improving nursing/clinical knowledge, skills, and competencies and in collaboration with others, supporting the ongoing professional development of clinical staff. Education methods will be offered to best meet the learning style of nurses and other clinicians and incorporates the principles of adult learning to optimize staff experience.· The CRN will collaborate and participate in the National Clinical, Quality, Health Information & Privacy team activities where appropriate to advance Professional Practice and support Corporate initiatives and will participate in the BPSO Champion and other Professional Practice Councils and Networks.· The CRN is responsible for education and staff development, particularly in relation to nurses. The CRN will incorporate evidence-based best practices, including BPSO Clinical Practice Guidelines. They will incorporate inter-professional core competencies where applicable. They will provide timely feedback to nursing/clinical staff on their learning and application to their practice competencies.· The CRN is responsible for the design and implementation of an education plan to meet the specific learning needs of staff in relation to their patient population. This includes the evaluation of the effectiveness of education, and the revision of education and learning activities to support changes and emerging clinical trends. This includes orientation and onboarding, and the development of skills and ongoing skill competency (validation of competency) for vascular access and infusion therapy and other clinical skills as required.· The CRN supports and participates in the implementation of quality improvement initiatives, while adhering to Bayshore policies and procedures.· The CRN will accompany primary care providers on home visits to assist in development of individualized care plans.· The CRN will work collaboratively with the interdisciplinary team (i.e. Palliative Care Consultation Team) and coordinate communication and resources among care providers within the circle of care.· The CRN will provide guidance, support and mentorship specific to clinical programs (e.g., wound care, palliative care) and the development of client centred care plan development and goal achievement and will provide support and guidance to Managers as requested or needed to ensure compliance with all relevant federal and provincial legislated standards of professional practice as well as internal policies.· The CRN will maintain privacy and confidentiality of client, employee, and corporate information.· The CRN will demonstrate knowledge of Standards of Practice guidelines of Provincial regulating body and will liaise with Provincial regulatory bodies as required.· The CRN will liaise with referring agencies and clients as necessary and ensure that identified client concerns and complaints are satisfactorily addressed and documented in a timely manner.· The CRN will lead the implementation of client satisfaction initiatives and participate in ongoing internal and/or external continuing education activities.· Complete other tasks as requested.SAFETY RESPONSIBILITIES· Comply with Bayshore’s Health and Safety policies and procedures, safe work practices and requirements.· Take reasonable care to work safely and carry out duties as to not cause accidental injury to self, fellow employees, or the general public.· Monitor compliance with health and safety policies, procedures, and standards.· Communicate all health and safety information including, but not limited to, policies, procedures, standards, hazards, hazard control and safe work practices.· Participates in proactive Health & Safety activities while performing all duties.· Immediately report all injuries, near misses or potential hazards to direct supervisor or alternate as appropriate.· Ensure all personal safety equipment (PPE) is being used properly; maintained and report defective or missing PPE to direct supervisor or alternate as appropriate.· Meets infection prevention and control requirements by adhering to infection control guidelines and practices and procedures.· Use appropriate lifting and transfer techniques.· Handle and dispose of hazardous materials according to WHMIS and organizational policies and guidelines.· Use equipment and supplies according to established standards and procedures.· Always maintain and promote a safe working environment.· Maintain awareness of role and participate in emergency response and/or drills a required.· Review and maintain all workplace harassment and/or violence policies and expectations.REPORTING RELATIONSHIPSThe Clinical Resource Nurse reports directly to the Area Director/Location Leader, Director of Clinical Management, or designate.QualificationsQUALIFICATIONSEducation & TrainingGraduated from an accredited School of Nursing; preferably with a Bachelor of Nursing degree; and current registration with the provincial regulating body. Additional training and certification will be required for this role and will be dependent on the types of clinical care performed by the location/branch. Examples include advanced palliative care education and/or certification (CAPCE, CHPCN), vascular access and infusion therapy certification (CVAA), and wound therapy education.ExperienceMinimum of five years community nursing experience or as determined appropriate for branch needs based on other skills/qualifications.Required Skills and AbilitiesExcellent interpersonal and leadership skills.Excellent verbal/written communication, reporting, and coordination skills.Demonstrated ability to teach, mentor, provide consultation and collaboration.Efficient with MS office and technology functions.Valid driver’s license and evidence of CPR certificationAbility to work competently in Palliative care, Infusion Therapy and Advanced Wound care planning.STANDARDS OF PERFORMANCEThe Clinical Resource Nurse must demonstrate ongoing competency in completing all duties and responsibilities, and maintaining skills and abilities as detailed in this job description, in response to changes by the regulating body, as well as agreed upon specific goals and objectives.
