Job description: Working together to provide outstanding integrated health care to our communities delivering best patient outcomes with exemplary standards and compassion, Muskoka Algonquin Healthcare (MAHC) is a multi-site organization that includes Huntsville District Memorial Hospital Site and South Muskoka Memorial Hospital Site.ROLEThe Nurse Practitioner in Complex Continuing Care will have an interest and focus in Geriatrics, Rehabilitation, and chronic disease management and function in an expanded nursing role demonstrating a high level of autonomy and expert skill to formulate clinical decisions and appropriately manage in collaboration with Physicians, the care needs of the patients within the Regional Complex Continuing Care Program. The Nurse Practitioner is part of the leadership team for the Complex Continuing Care Program, providing leadership in knowledge-based practice, the development of standards, processes, and quality initiative enhancing the care for Complex Continuing Care patients.

  • Will establish Medical Directives with Physician leaders in the CCC Program to support practice.
  • Performs a comprehensive or symptom-directed assessment and synthesizes data from multiple sources to establish a plan of care for the patient.
  • Keeps complete, accurate, and timely records of patient consults which will provide information to assist other practitioners in continuing to meet the patient’s ongoing care needs.
  • Liaise/collaborate with interdisciplinary colleagues to formulate strategies ensuring an effective and efficient discharge from hospital.
  • Assesses patients ensuring that they have a diagnosis, a plan of care with smart goals, and their admitting acute conditions are resolved.
  • Initiates and manages the care of patients with diseases or disorders within the NP scope of practice and/or monitors the ongoing therapy of patients with chronic stable illness. Consults Most Responsible Physician or Hospitalist when patient destabilizes.
  • Consults with patient/family, nursing staff, Rehabilitation staff, Physicians, and other members of the health care team to develop comprehensive care plans for individual patients by attending patient care rounds and family meetings as required.
  • Participates in appropriate Hospital committees relevant to patient care issues, including individual, family, community, and professional issues.
  • Participates in strategic planning and program development.
  • Assists with the integration of best practice guidelines and current evidence-based research into clinical practice.

REQUIRED EDUCATION & CERTIFICATION(S)

  • BScN required, master’s degree in nursing.
  • Registered Nurse – Extended Class or completion of an Acute Care Nurse Practitioner Training Program. The Nurse Practitioner must maintain all standards as required by the College of Nurses of Ontario with regard to Extended Class Certification.
  • Current certificate of registration in good standing with the College of Nurses of Ontario.
  • Current registration in the Registered Nurses Association of Ontario (RNAO) and Canadian Nurses Protective Society (CNPS) or Nurse Insure.
  • Must be a member of the R.N.A.O.
  • Must attain and maintain certification in B.L.S..

REQUIRED SKILLS & EXPERIENCE(S)

  • Recent clinical experience as a nurse in a hospital setting with a rehabilitation focus with the geriatric population preferred.
  • Experience with the development of Medical Directives, care pathways, and clinical practice guidelines to support clinical practice.
  • Comprehensive knowledge of best practices with a rehabilitation focus for the geriatric population and knowledge of the Senior Friendly Care Framework.
  • Proven leadership skills.
  • Strong organizational, analytical, conceptual, and communication skills.
  • Excellent attendance record.
  • Proven success in providing inspirational leadership in a team environment and ability to encourage innovation and creativity amongst team members.
  • Ability to work collaboratively with the Interdisciplinary and Physician Team.
  • Commitment to the vision, mission, and values.

DESIRED EDUCATION & CERTIFICATION(S)

  • Canadian Nurses Association Gerontology Certificate preferred.
