Job description: POSITION SUMMARY:
The Schizophrenia Outpatient Clinic (SOC) and Transitional Outpatient Program for the Schizophrenia Service (TOPSS) utilizes a clinical case management model of service delivery that strives to support individuals with schizophrenia and experiences with psychosis to regain and manage their health, and to live meaningful lives through the provision of compassionate, innovative, specialized mental health and addiction services.The Nurse Practitioner (NP-PHC) is an autonomous health care provider who works in collaboration with physicians, other members of the interdisciplinary team and clients/families to provide evidence-informed and integrated care to registered outpatients of TOPSS and SOC. The NP’s practice is grounded in professional, ethical and legal standards using advanced nursing knowledge, skills, experience, clinical reasoning, problem solving and critical thinking. The NP builds and expands on established RN competencies with an expanded scope of practice based on legislative authority to autonomously perform the controlled acts. The NP’s fundamental competencies include clinical practice, leadership, research, consultation, and collaboration. All duties and responsibilities of the NP will be consistent with the Standards of Behaviour and policies set by St. Joseph’s Healthcare Hamilton. The NP will report jointly to the Manager of SOC/ TOPSS and the Director of Nursing.QUALIFICATIONS
-Current registration with the College of Nurses of Ontario in the RN-Extended Class with specialty certification as NP – Primary Health Care is required
-Master’s Degree in Nursing required
-Proof of successful completion of controlled substances education required
-Faculty appointment or eligibility for appointment with a School of Nursing in an academic institution
-CNA certification in Psychiatric/ Mental Health Nursing (CPMHN(c)) preferred
-Demonstrated experience with psychiatric assessment.
-Providing pharmacologic and non-pharmacologic interventions
-At least three years recent demonstrated experience as a Nurse Practitioner with experience in community based setting servicing individuals diagnosed with schizophrenia and related psychotic disorders.
-Demonstrated clinical expertise and experience working with clients experiencing mental illness and/or addiction required
-Collaboration and demonstrated knowledge with relevant community supports and resources that service individuals diagnosed with a mental health issues
-RNAO, NPAO, CNA membership preferred
-Current BCLS/HCP Certification required
-Comprehensive knowledge of the Mental Health Act, Health Care Consent Act and other related legislation in Ontario required
-Demonstrated knowledge of legislative and ethical responsibilities relating to scope of practice and healthcare required
-Demonstrated self-directed decision making and problem solving required
-Demonstrated excellent interpersonal skills and ability to maintain effective collegial relationships and teamwork required
-Demonstrated knowledge sensitivity and expertise in care of seniors and their families
-Gentle Persuasive Approach training preferred
-Demonstrated effective conflict management/ resolution skills required
-Demonstrated scholarly inquiry, knowledge translation and effective research skills
-Ability to minimize risk adhering to established policies and procedures
-Demonstrated proficiency in computer skills (ie: PDM, Clinical Connect, MS Office/ Outlook , EPIC, GENCAN etc.) required.
-Demonstrated data management for evaluation, interpretation and process change
