As a progressive and vibrant healthcare organization, we are committed to being an innovative center of excellence in community hospital care.Job DescriptionReporting directly to the Director of Nephrology at Halton Healthcare the Nephrology Patient Care Manager works in collaboration with the other Nephrology Patient Care Managers, the Program Medical Leadership, and Professional Practice Clinician to implement clinical program priorities and oversee the daily operations and professional practice of the Nephrology and Dialysis services.The Patient Care Manager plans, implements and evaluates activities related to the provision of high quality patient care services and programs, selects, hires and develops staff, works collaboratively with the team, and maintains fiscal responsibility.The Patient Care Manager is also be instrumental in leading the program’s work in the key priorities of the Ontario Renal Network and in supporting partnerships with the ORN, the Interchange project, community partners, and Central Region.QualificationsCurrent Registration with the College of Nurses of Ontario· BScN required· Masters preparation or evidence of substantial completion preferred· A minimum of 3 years of recent experience as a patient care manager in a similar hospital environment· Excellent human resource management skills including recruitment and retention of staff· Excellent budgeting and financial management skills· Proven leadership skills preferably in Nephrology· Certification in Nephrology an asset.· Excellent computer skills including proficiency with Microsoft Office applications including Word, Excel and PowerPoint· Superior verbal and written communication skills· Excellent customer service skills· Demonstrated ability to support team building· Must be able to meet the physical demands of the position· Demonstrated commitment to continuing education· Satisfactory employment and attendance record requiredAdditional InformationHours: Monday to Friday – 7.5 hour day shifts (Hours are subject to change based on operational needs)Salary: $56.369 – $70.462 per hourDate Posted: August 20, 2024Internal Applicants Due By: August 28, 2024REF873MHalton Healthcare is an equal opportunity employer who is committed to integrating diversity, equity and inclusion throughout our operations, policies and culture.
