As a progressive and vibrant healthcare organization, we are committed to being an innovative center of excellence in community hospital care.Job DescriptionThe Rehabilitation and Geriatrics Program encompasses an array of inpatient and outpatient services at Georgetown Hospital (GH), Milton District Hospital (MDH), and Oakville Trafalgar Memorial Hospital (OTMH). Together they address the complex needs of the adult population requiring specialized care, general rehabilitation and/or reactivation within the Halton Healthcare catchment area.The Patient Care Manager for Rehabilitation Allied Health will provide leadership to Allied Health staff (Physiotherapy, Occupational Therapy, Speech Language Pathology and Recreation Therapy) across the 3 hospital sites in inpatient areas. The role will work collaboratively with the Patient Care Managers of inpatient units to synergistically achieve exemplary patient experiences always.This is what you’ll be doing:The Manager of Rehab Services provides administrative and clinical leadership in the delivery of high-quality, patient-centered care within the Rehabilitation and Geriatrics Program· Responsible for the clinical and resource management of Rehabilitation professionals within all Acute and Complex Continuing Care patient care areas across the 3 hospital sites· Creates an environment which fosters a collaborative, interdisciplinary approach to patient focused care, innovative thinking, continuous learning and empowerment of autonomous decision making· Facilitates optimal care and high patient, family and staff satisfaction through coordination and collaboration with patients, families and staff and continuously seeks to improve and sustain a standard of best practice based on current evidence· Participates in the implementation of the related clinical program priorities and corporate activities identified by the Rehab and Geriatrics Program Planning Committee and hospital management· Participates as required in regional Mississauga Halton Local Health Integrated Network (LHIN) initiatives and committees· Promotes the staff’s involvement in the maintenance of a safe workplace through Committee involvement and safety champions· Participates with the Program Director (PD) in the development of an annual operating plan, for Rehab Services including staffing, supply and capital budget, which ensures the achievement of clinical program priorities, within available resources· Participates with the PD in the development of appropriate risk and quality indicators for their services/programs that measures the service accessibility, quality of care, and family satisfaction· Monitors program quality indicators and implements continuous quality improvement initiatives as indicated to ensure quality patient-focused care· Communicates regularly with physicians and Patient Care Managers to obtain their input and support for patient care and program initiatives; facilitates cross-site communication, collaboration and teamwork amongst front-line staff to establish and maintain standardization of service/program policies, procedures and practices as appropriateQualificationsHalton Healthcare endeavors to hire individuals who are competent in their chosen profession and who exemplify our mission, vision and values of accountability, respect and compassion which guide our individual behaviours and attitudes.· Regulated Allied health professional with a Baccalaureate degree required; Masters preferred· Completion of an accredited post-grad program in Health or Business administration (or in the process of earning same) an asset· A minimum of three to five years’ experience in a supervisory/leadership capacity in a rehab environment required· Relevant clinical rehabilitation management skills are essential· Budgetary preparation and management experience an asset· Experience in proposal writing Membership on relevant regional committees – (i.e. West GTA Stroke Network, GTA Rehab. Network, Provincial Bone & Joint Network.
