Job description: SE Health is Hiring a Full Time Personal Support Workers (PSW) – Journey Home HospiceMost Canadians want to die at home, but where do you go to die when you don’t have a home? This is a question that Journey Home Hospice – operated by Saint Elizabeth Foundation – is answering for patients experiencing homelessness and structural vulnerability. With 13-beds in the heart of downtown Toronto, four beds in Windsor, and an international, teaching and learning program, we are creating specialized trauma-informed, culturally safe, and harm-reducing approaches to care that are innovating care options for people in need in Canada and beyond. REQUIREMENTS:
- Current PSW Certificate mandatory and availability to work flexible shifts (Days & Nights)
- Direct experience working with Mental Health patients and/or homeless populations preferred
- Hospice Palliative care experience and ongoing continuing education is an asset
- Experience working with vulnerable populations
- Excellent Communication skills both orally and verbally along with exceptional computer skills
- Well-developed interpersonal skills and excellent organizational skills
- Able to work independently as well as part of an integrated team (nurse, physician, hospices, etc)
- Effective problem-solving and conflict management skills
- A current CPR and Standard First Aid certificate is required
