As an independent program, we operate within a collaborative framework alongside SPRINT Senior Care, who takes the administrative lead, and VHA Home HealthCare, who provides contracted Physiotherapy and Allied Health support. Our goal is to support frail, homebound seniors living at home for as long as possible, providing comprehensive and dignified care that includes palliative services. The successful candidate will be joining a cohesive and supportive team that has been providing excellent care in the community for over 17 years! #LI-VHAFind more about this program on : www.seniorshousecalls.caKey Areas of Accountability:VHA’s Physiotherapist have the following key areas of accountability, but not limited to:

  • Assesses clients using interviews, standardized and informal assessment techniques in accordance with professional practice guidelines and standards established by VHA.
  • Implements client interventions based on assessment information, client input and in conjunction with the client’s integrated care plan.
  • Ensures that services provided are best practice, evidence-based and evaluated through the routine use of valid and reliable outcome measured.
  • Attends client related meetings to report on assessment findings and discuss plans for intervention, progress reports and discharge as required.
  • Ensures coordinated delivery of care through ongoing communication with all members of the client’s team regarding plans and schedules for interventions.
  • Documents all assessment findings, treatment plans, progress and discharge summaries of interventions as required by the profession, referral source and the organization.
  • Prepare statistical records of client care and related activities as required by VHA, the Ministry of Health and other agencies.
  • Adheres to the code of ethics of the profession, in maintaining standards of practice for the profession as defined by the Ontario College of Physiotherapists.

Required Qualifications:

  • Baccalaureate or Masters degree in Physiotherapy from a recognized school of physiotherapy.
  • Registered, in good standing, with the College of Physiotherapists of Ontario.
  • Minimum of 2 years clinical experience (asset)
  • Specialized courses to support areas of clinical expertise.
  • Clinical expertise to provide a variety of physiotherapy interventions.
  • Knowledge of community care requirements and resources available.
  • Excellent consultation and teaching skills, and utilizes an evidence base to clinical practice.
  • Familiar with the routine use of valid and reliable outcome measures to support clinical interventions.
