.The Registered Practical Nurse (RPN) is part of a team that provides direct care, education and support to older adults living in a long-term care home (LTCH); transitioning into or between healthcare and/or long-term care settings within the geographical boundaries of the South East Local Health Integration Network (SELHIN). Clients include those who are demonstrating responsive behaviours associated with the various types of dementia, delirium, mental health, addictions, or other neurological conditions.The RPN is responsible for developing and articulating strategies for the management of responsive behaviours using information collected through assessment and collaboration with the RN and PSWs. The RPN will provide ongoing monitoring of the client; recognizes changes in client status; probes further and consults appropriately with the RN or other healthcare team members, to make adjustments to the Collaborative Care Plan. The RPN develops plans of care to achieve identified client goals when overall care needs are less complex, outcomes are predictable and risk of negative outcomes is low. The RPN works in consultation with the RN and others to meet care needs of more complex clients. Additionally, the RPN will liaise with community agencies, other practitioners and key partners to coordinate care for clients. The RPN will also contribute to program development and participate in the research activities of the program. The RPNs work is conducted in a manner that is consistent with the Mission, Vision and Values of Providence Care Centre.Required Qualifications and Experience
- Current unrestricted registration as an RPN with the College of Nurses of Ontario
- RPN Diploma (with medication certificate or full scope of practice) from an accredited college
- Current CPR and First Aid Certificates
- Minimum three years of related experience in providing support to older adults with responsive behaviours related to dementia mental illness, addictions, or other neurological conditions.
- I.E.C.E.S. TM, Gentle Persuasive Approaches, U-First! And Montessori training an asset
- Satisfactory criminal-record and vulnerable-sector screening
- Experience in the management of responsive behaviours an asset
- Certificate in dementia care (or equivalent) from an accredited educational institution, preferred
- Extensive experience working in various care settings working with residents who demonstrate responsive behaviours in conjunction with complex care needs.
- Strong written and oral communication skills
- Experience with quality-improvement processes an asset
- Prior experience working in collaborative practice settings
- Ability to work effectively with others in an interprofessional care team
Position Responsibilities
- Utilizes evidence-based screening and observation tools during assessment with referred individuals, family members and primary caregivers;
- Supports clients in the development and implementation of a Collaborative Care Plan tailored to the client’s individual needs in order to improve quality of life and assist in the management of responsive behaviours;
- Provides ongoing review, reassessment and revision of Collaborative Care Plan in consultation with the LTC Home team and the MRT RN for more complex clients
- Provides timely and accurate documentation including assessments, behavioural support plans, and progress notes as required and in accordance to the College of Nurses of Ontario.
- Evaluates care outcomes and ensures that care provided is accurately documented
- Works collaboratively with other Behavioural Support Services operated by Providence Care: Seniors Mental Health Outreach Team and Psychogeriatric Resource Consultants
