As a progressive and vibrant healthcare organization, we are committed to being an innovative center of excellence in community hospital care.Job DescriptionAs an integral member of the interdisciplinary Step-Up Neuro Rehab team, the social worker will assess, plan, implement and evaluate patient needs as well as work with their family and/or significant others. The social worker will enable stroke/neuro clients in their reintegration within their community setting by providing critical thinking and problem solving. The social worker will enable reintegration by focusing on the needs and goals of the patient and family. Will advocate within both the healthcare setting and community for the removal of systemic barriers that negatively influence the delivery of healthcare and the continuity of care using the appropriate resources in partnership with external partners.
- Conduct comprehensive social work assessments using a variety of modalities and in accordance with professional standards and clinical practices and policies.
- Consider the individual needs and readiness of patient/family/caregiver when providing resources and connecting them to community services
- Provide education to stroke/neuro patients and their families regarding what to expect as they transition back into the community
- Collaborate with the inter-disciplinary team to address ongoing patient needs, to participate in discussions to help optimize patient care and to ensure the coordination of community based support services.
- Engage patient to assist with adjustments related to socio/emotional health issues and identify patient’s network of supports
- Formulate a care plan to accomplish patient’s goals
- Resolve complex issues with patient/family through use of conflict resolution, providing mediation and advocacy both with internal and external partners

- BSW or MSW required.
- Current registration and membership in good standing with the College of Social Workers and Social Service Workers
- Minimum 3 years experience in a hospital or community health organization.
- Experience working in neurology/stroke setting preferred
- Demonstrates strong communication and interpersonal skills with the ability to work collaboratively with patients/families, interprofessional team members, and community stakeholders through face to face or telephone interventions, class teaching, group facilitation, adult learning
- Skillful implementation of planned social work interventions, both individually and in group settings, with the patient, family, caregiver, inter-disciplinary team and community resources
- Able to address the unique health, social and service challenges for the patient/family/caregiver in keeping with Canadian Stroke Best Practice Guidelines
- Demonstrates a high degree of initiative, self-direction and ability to multitask in a fast paced environment
- Demonstrated ability to function effectively as a member of the health care team
- Satisfactory employment and attendance record
- Comprehensive knowledge of community resources and research skills to secure new resources.
- Care passionately about the needs and well-being of patients, families and caregivers and the needs of our local community
